Sleeping Safely After Hip Replacement: Dos and Don'ts

Sleep comfortably without risking your new hip. Learn the best sleeping positions after hip replacement surgery and how long to use a pillow between your legs.

Sleeping Safely After Hip Replacement: Dos and Don'ts

Protecting your new hip while you sleep is critical in the first few weeks.

During deep sleep, you have no control over your muscles. If you accidentally cross your legs or roll incorrectly, you could risk a dislocation.

Here is how to sleep safely and soundly.

The Best Position: On Your Back

For the first 4-6 weeks, sleeping on your back is the safest option.

  • The "V" Shape: Keep your legs slightly apart.
  • Use a Pillow: Most surgeons provide a triangular foam wedge (abduction pillow) or ask you to place a regular pillow between your thighs. This prevents your legs from crossing.

Can I Sleep on My Side?

The Non-Operated Side

  • You can usually roll onto your "good" side after a few weeks.
  • CRITICAL: You MUST keep a thick pillow between your knees.
  • This pillow acts as a spacer. Without it, the top leg (operated leg) will drop across your body, twisting the hip joint into a danger zone.

The Operated Side

  • Sleeping on the incision site might be uncomfortable for 4-6 weeks. Once the soreness fades, it is safe.

Positions to Avoid

  1. Stomach Sleeping: Generally avoided as it twists the neck and hips.
  2. Crossing Ankles: Never cross your ankles while lying on your back.
  3. Drawing Knees to Chest: Do not curl up into a tight fetal ball in the early weeks (past 90 degrees flexion).

When Can I Stop Using the Pillow?

Usually after 6 weeks. By then, the capsule (tissue around the hip) has tightened, and the risk of dislocation drops significantly. Ask Dr. Kale for clearance before ditching the pillow.

Takeaway

A simple pillow is your best defence against dislocation at night.

Respect the precautions for a few weeks, and you will enjoy a lifetime of safe sleeping.
